During this time we will be working closely together to set up and configure your Renaissance Protection Plan. In order for this process to move as quickly as possible, we will need you to fill out and return the following required information:

• Protection Plan Pricing
• Retailer Setup Packet
• W-9 Request for Taxpayer Identification Number and Certification
• Credit Application & Credit Card Authorization Form
  (This is needed for setup in our accounting system and to process a one-time setup fee of $100)

Once setup is complete and your plan is active, you will receive POP collateral to assist you in marketing your Renaissance Protection Plans to your customers. This POP collateral kit consists of:

• (1) Spider Stand Sign
• (50) Table Tents
• (200) Slim Jim Pamphlets
